Patch removing references to deprecated constants in man3 pages.

Some pages still mention constants which have been deprecated.

For example:
- GL_LINE_WIDTH_GRANULARITY (SMOOTH_LINE... should be used)
- GL_LINE_WIDTH_RANGE (SMOOTH_LINE... should be used)
- GL_MAX_VARYING_FLOATS (GL_MAX_VARYING_COMPONENTS should be used)
- GL_VERTEX_PROGRAM_POINT_SIZE (GL_PROGRAM_POINT_SIZE should be used)
- GL_POINT_SIZE_MIN
- GL_POINT_SIZE_MAX
- GL_POLYGON_MODE
- GL_INDEX_SHIFT
- GL_INDEX_OFFSET
- GL_INDEX_MODE
- GL_MAP_STENCIL
- GL_PIXEL_MAP_S_TO_S
- GL_*_SCALE
- GL_*_BIAS

Fixed this (I think). A couple of these are deprecated, but still in core. Otherwise, good catch. File a new bug if I missed anything.
